Maryam Reproductive Health + Wellness

At Maryam Reproductive Health + Wellness, we value your time and strive to provide accessible, high-quality, and personalized care. To ensure fair scheduling and availability for all patients, we maintain the following Refund and Cancellation Policy.

Appointment Cancellations and Rescheduling

Patients may cancel or reschedule appointments without penalty by providing at least 24 hours' notice prior to the scheduled appointment time.

If a cancellation is made 24 hours or more before the scheduled appointment, the patient may choose one of the following options:

• A full refund of the amount paid for the appointment; or

• Application of the payment as a credit toward a future service at Maryam Reproductive Health + Wellness.

Refunds will be issued to the original form of payment and may take up to 10 business days to process, depending on the financial institution.

Service credits:

• Are non-transferable

• Have no cash value

• Must be used within twelve (12) months of the original appointment date unless otherwise specified by the Practice

• May only be applied to services provided directly by Maryam Reproductive Health + Wellness

Late Cancellations and No-Shows

The following are not eligible for refunds or service credits:

• Cancellations made less than 24 hours before the scheduled appointment

• Same-day cancellations

• Missed appointments ("no-shows")

• Failure to attend an appointment without notice

• Arrivals so late that the appointment cannot be completed in full, at the provider's discretion

In these circumstances, the full appointment fee will be retained by the Practice.

Late Arrivals

Patients who arrive late may be accommodated at the discretion of the provider based on the clinical schedule and time remaining for the appointment.

If the visit cannot be completed due to late arrival, the appointment may be treated as a late cancellation or no-show and will be subject to this policy.

Refund Policy

Except as expressly provided in this policy for appointments canceled with at least 24 hours' notice, payments made to Maryam Reproductive Health + Wellness are non-refundable.

No refunds will be issued for:

• Completed services

• Missed appointments

• Same-day cancellations

• Late cancellations

• Membership fees

• Unused membership benefits

• Expired prepaid services

• Administrative services

• Laboratory or diagnostic services already ordered or performed

Clinical Services

Fees paid for completed medical services, consultations, evaluations, procedures, telehealth visits, home visits, laboratory collections, forms, letters, care coordination, and administrative services are non-refundable once services have been rendered.

Membership Programs

For patients enrolled in concierge, membership-based, or subscription services:

• Membership fees are non-refundable once a membership period has begun.

• Memberships may be canceled at any time with written notice.

• Membership benefits will remain active through the end of the current billing cycle.

• No prorated refunds will be issued for unused services, unused benefits, or early termination of membership.

Membership fees are not health insurance and do not guarantee reimbursement for services not specifically included in the membership agreement.

Packages and Prepaid Services

Prepaid wellness packages, educational programs, service bundles, and other prepaid offerings are generally non-refundable.

If an appointment associated with a prepaid service is canceled at least 24 hours in advance, the patient may either:

• Reschedule the appointment;

• Receive a service credit for future use; or

• Request a refund if no services from the package have yet been utilized.

Unused prepaid services must be utilized within the validity period established by the Practice. Expired services are forfeited and are not eligible for refund.

Laboratory Testing and Third-Party Services

Fees paid for laboratory testing, pathology services, imaging studies, medications, supplements, genetic testing, specialty testing, or services provided by third-party vendors are non-refundable once ordered, processed, shipped, or performed.

Any costs paid to outside vendors are not eligible for service credits or refunds through the Practice.

Insurance and Patient Responsibility

Patients are responsible for understanding their insurance benefits, coverage limitations, deductibles, copayments, coinsurance obligations, and authorization requirements.

Refunds will not be issued for services denied by insurance carriers due to:

• Non-covered services

• Lack of prior authorization

• Coverage limitations

• Deductibles

• Coinsurance obligations

• Eligibility issues

• Failure to comply with insurance requirements

Verification of benefits by the Practice does not guarantee payment by an insurance carrier.

Insurance and Third-Party Payments

Payments made by insurance companies, health plans, flexible spending accounts (FSAs), health savings accounts (HSAs), or other third-party payers are subject to the terms and policies of those entities.

This Refund and Cancellation Policy applies to all patient-responsible balances, self-pay services, membership fees, and other direct payments made to Maryam Reproductive Health + Wellness.

In the event of a verified overpayment, duplicate payment, or billing error, the Practice will issue an appropriate refund or account credit.

Exceptions

We understand that emergencies and unforeseen circumstances may arise.

Requests for exceptions to this policy may be reviewed on a case-by-case basis at the sole discretion of Maryam Reproductive Health + Wellness. Approval of an exception in one instance does not obligate the Practice to grant exceptions in future circumstances.

Agreement

By scheduling an appointment, purchasing a service, enrolling in a membership program, or receiving care from Maryam Reproductive Health + Wellness, you acknowledge that you have read, understood, and agree to this Refund and Cancellation Policy.
