These Terms and Conditions ("Terms") govern your access to and use of the Maryam Reproductive Health + Wellness website, patient portal, online scheduling platform, telehealth services, mobile applications (if applicable), membership services, home visits, and any related digital or in-person services (collectively, the "Site" and "Services").

By accessing or using the Site or Services, scheduling appointments, communicating with our practice, enrolling in membership programs, or receiving care, you agree to be bound by these Terms, our Privacy Policy, and our Notice of Privacy Practices. If you do not agree with these Terms, please do not use the Site or Services.

Medical Emergency Disclaimer

IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 OR SEEK IMMEDIATE EMERGENCY MEDICAL CARE.

The Site and Services are not intended for emergency medical situations and should not be used for urgent or life-threatening concerns.

Our Relationship With You

Maryam Reproductive Health + Wellness provides obstetric, gynecologic, reproductive, wellness, and integrative healthcare services through licensed healthcare professionals.

Information provided through the Site, Services, social media, educational content, and electronic communications is for informational and educational purposes only and is not medical advice.

Use of the Site alone does not establish a provider-patient relationship. A formal provider-patient relationship is established only after registration, informed consent, and evaluation by a licensed provider.

Patient Responsibilities

By using the Site and Services, you agree to:

• Provide accurate, complete, and current information

• Maintain confidentiality of your account credentials

• Use the Site and Services only for lawful purposes

• Notify us of any unauthorized account access

• Follow all applicable laws and regulations

You are responsible for obtaining and maintaining all necessary devices, software, and internet access required to use the Site.

• Telehealth is not appropriate for emergencies

• Providers may determine telehealth is not clinically appropriate

All telehealth services are provided by licensed healthcare professionals within their scope of practice.

Concierge Care, Home Visits, Communication Expectations, and After-Hours Services

Maryam Reproductive Health + Wellness may offer concierge, membership-based, home visit, and enhanced access services designed to provide a more personalized healthcare experience.

Direct Communication with Providers

Membership may include access to secure messaging, phone calls, email, text messaging, or other direct communication methods.

Response times are not guaranteed. Communications received outside of business hours, weekends, or holidays may be addressed on the next business day or as clinically appropriate.

Electronic communication is not appropriate for emergencies or urgent medical concerns.

Home Visits

Home visits may be offered for select obstetric, gynecologic, postpartum, newborn, wellness, and other services.

Home visits are subject to provider discretion, geographic limitations, scheduling availability, and clinical appropriateness.

The Practice may determine that an in-office, hospital-based, or emergency evaluation is necessary.

Home visits are not emergency services.

Membership Services

Membership programs may include services such as:

• Extended visits

• Enhanced provider access

• Preventive and wellness care

• Care coordination

• Home visits

• Telehealth visits

• Educational and wellness support

Membership benefits are defined in the applicable membership agreement and may be modified by the Practice with reasonable notice.

Membership does not guarantee unlimited services or immediate provider availability.

After-Hours Availability

The Practice does not guarantee 24/7 provider availability unless explicitly stated in a written agreement.

After-hours communication is intended for non-emergent concerns and may be addressed based on urgency and provider availability.

No Substitute for Emergency Care

All communication methods, including messaging, telehealth, and home visits, are not substitutes for emergency medical care.

Patients must seek emergency care for serious or life-threatening conditions, including but not limited to:

• Chest pain or difficulty breathing

• Severe bleeding

• Signs of stroke

• Loss of consciousness

• Severe abdominal or pelvic pain

• Obstetric emergencies

• Any condition requiring immediate attention

Membership Is Not Insurance

Membership fees are not health insurance and do not replace comprehensive medical coverage.

Membership covers only specified services and does not guarantee coverage for all medical needs, testing, hospitalizations, medications, or specialty care.

Payment Responsibility

Patients are responsible for all fees associated with care, including deductibles, copayments, coinsurance, self-pay charges, membership fees, and non-covered services.

Payment may be required at the time of service.

Insurance

Insurance coverage is between the patient and their insurance carrier. The Practice does not guarantee reimbursement and is not responsible for insurance coverage determinations.

Privacy and Health Information

All personal and health information is handled in accordance with our Privacy Policy and Notice of Privacy Practices, including HIPAA regulations.

Third-Party Links

The Site may contain links to third-party websites. We are not responsible for their content, security, or privacy practices.

Intellectual Property

All content on the Site is owned by Maryam Reproductive Health + Wellness or its licensors and is protected by intellectual property laws.

No content may be copied, reproduced, or used without written permission.

Prohibited Uses

Users agree not to:
